2016 REMINDER NOTICE TO ANIMAL OWNERS
DOG and CAT Licenses are due for Renewal BY January 31st
Dogs and Cats over 6 months of age are required to be licensed annually.
Please notify this office if you have moved or no longer own this animal at: 7 3 2 – 8 9 2 – 4 1 8 3
PROOF FROM VET IS REQUIRED:
IF YOUR PET HAS BEEN SPAY/NEUTERED SINCE OBTAINING YOUR LAST LICENSE or
IF RABIES VACCINE HAS EXPIRED or NEEDS TO BE UPDATED **
** STATE REGULATIONS REQUIRE THAT ALL RABIES IMMUNIZATION MUST BE VALID THROUGH NOVEMBER 1st. IF THE VACCINATION EXPIRES BEFORE NOVEMBER 1st 2016 YOU MUST GET A BOOSTER PRIOR TO LICENSING.
