Certified Copies of Vital Records for events that took place in Point Pleasant Borough may be obtained IN PERSON or BY MAIL.
FEE: $20 Initial Certified Copy
Additional Copies of same record ordered at the same time $5.00 per copy

A vital record can only be issued to the subject of the record, who must be 18 years old or to the subject’s parent, legal guardian, legal representative, spouse, child, grandchild or sibling of legal age.
Proof of connection to the person of the vital record must be submitted with the request.

VALID photo driver’s license with current residential address.
OR
VALID non-photo driver’s license and an alternate form of ID with current residential address.
OR
Two alternate forms of ID, showing current residential address and dated within the last 3 months.
